This is a partial run that I do in act 3 that is a balance between both paragon exp and item farm. I put the minimum amount of maps in this run as possible in order to make it an efficient EXP and item farm guide. By all means, I try to emphasize that if you have more time on your hands, feel free to do the following areas as well. This is a skeletal run, that really is supplemented by more maps given more time. You might say its like a build-a-run! Choose what fits your time arrangements. I only set the 5 maps mandatory below because they get your NV stacks the fastest without sacrificing much potential loot / EXP. Otherwise, the supplementary maps that should be visited I arrange by priority:
High Priority: These have a lot of elites and also a high density of white trash mobs. They are very efficient and should be a part of a longer run.
Medium Priority: This only includes Stonefort. As Stonefort does yield some packs of trash mobs and elites, it is simply too spaced between mobs and elites for it to be very viable. Do if you have extra time.
Low Priority: This only includes Azmodan himself. Albiet a boss, he only guarantees 2 rare drops, but it takes a while for him to kill, and its not worth the time for those 2 extra drops and little experience. It is much more worthwhile to restart another run and get stacks so you can farm trash and elites, rather than killing Azmodan. Unless you just have free time and want to do it for fun, then go right ahead.

Start with Heart of Sin - Kill Azmodan
The places I go to in this run are (in this order):
The Tower of the Damned - 0:11 The Tower of the Cursed - 2:19 The Core of Arreat - 5:15 The Keep Depths Level X - 7:46 Bridge of Korsikk - Fields of Slaughter - 12:58
Again to reiterate, feel free at this point to add any of the above maps to the run to suit the time you have for a run. You may ask, even if I have time on my hands, is it more efficient to do many of these small runs, or the longer ones? I would say it depends on what you are trying to achieve. For EXP, you want as many iterations of Fields of Slaughter as possible, so small runs would be more beneficial. But for loot, you would want to explore more around Act 3 with your 5 stacks, so a longer run might be more suitable. Even with these 5 maps and the 5 high priority maps in one run, you don't sacrifice much EXP either, to find some more items. So I tend to lean towards the longer runs when I can.
The total amount of experience I got during this run (with a 25% EXP gem in helm) was 4,320,000 (3,240,000 EXP without a gem). A decent amount, and also I got about 10 worthy unID rares (iLvl61+).
